Private Pay Clients:

These are clients who typically pay “out of pocket” for transportation service to and from medical appointments as well as private contracts and service agreements you cultivate with key facilities.

In building your senior transportation service, it is my hope that you build a strong foundation of private pay, contracted, and service agreement work because the rates of reimbursement tend to be higher as compared to other sources of reimbursement.

Further, private pay work empowers your service because it affords you more freedom and flexibility to establish your own terms versus having to comply with various forms of regulation from government agencies or entities.

As you will learn in my material, I encourage you to build a diverse and dynamic business model with multiple sources of profitable revenue, but if the rates of reimbursement are not profitable, you don?t provide service!

I continue to work with a growing number of client-providers who only provide private pay, contracted, and service agreement work because of the higher rates of reimbursement. For them, pursing lower forms of reimbursement simply are not worth their time. They are simply too busy cultivating private pay clients.

Private pay, contracted, and service agreement will strengthen the revenue and increase the profit margin of your medical transportation business.



Medicaid/Brokers:

In addition to receiving free healthcare, according to Article 1915 of the Social Security Act, qualifying Medicaid recipients also receive free non-emergency medical transportation to and from medical appointments. The logic for free Medicaid transportation service is out of both necessity and preventive care.

Obviously, Medicaid will not reimburse for personal errands or outings, but for medical related appointments, treatments, and therapies, transportation providers can make a lot of money.

Medicaid reimbursements are comprised of both state and federal funds. Therefore, as you can imagine, the processes, procedures, and rates of reimbursement will vary by state and even region.

An increasing number of states outsource some, if not all, of their Medicaid transportation processes to Medicaid brokers in an effort to reduce cost. Some states have multiple Medicaid brokers operating in their state.

In accordance with state mandates, Medicaid brokers establish policies and procedures and subcontract to transportation providers via Broker Agreements. The Medicaid broker is responsible for coordinating all Medicaid transportation with transportation providers in addition to overseeing provider fleet management.

Although Medicaid reimbursements can be very consistent, their per-trip rates of reimbursement tend to be lower as compared to private pay, private contracts, and private service agreement work.


Workers
Compensation Insurance:

This type of reimbursement is for the transportation of people hurt “on the job” and needing to get to and from physical therapy and related medical appointments. Workers Comp is one of the few forms of insurance that reimburses for non-emergency medical transportation and can be a great source of revenue.

Rates of reimbursement for workers compensation are typically higher as compared to Medicaid transportation, but not as much as private and contracted transportation.

Some workers compensation policies are managed and reimbursed directly through the underwriter while others are management and reimbursed through brokers.

Hospitals:

The vast majority of hospitals outsource assisted transportation needs to local vendors and providers. They are critical strategic partners for your business and offer a wide variety of non-emergency medical transportation opportunities.

Hospitals provide “general” discharges and emergency room discharges. They can also provide inpatient transportation to and from other facilities and doctor offices for specific test and treatments outside the hospital.

For senior transportation providers serious about building a legitimate business that can be grown, scaled, and sold for a future profit, establishing strong working relationships with hospitals and their supply chain management is a must.

In order to ensure adequate coverage and access to reliable wheelchair and stretcher transportation for their patients, a growing number of hospitals and emergency departments are willing to consider contracts and service agreements with area providers.


Facility Care:

There are a wide variety of facilities offering different levels of care that require assisted transportation to meet the needs of their residents. Skilled care and rehabilitation facilities are a “staple” for the non-emergency medical transportation industry. Why? Because they are literally caring for your most important niche market – the elderly population!

Other facilities and retirement communities offering assisted and independent living services might provide transportation with their own vehicle(s), but frequently their service only operates a few days per week and during specific times. Therefore, it is very possible to position your senior transportation service as a “support” or “secondary” solution to further accommodate resident needs.

Like hospitals, skilled care, rehab, and even assisted living facilities can provide you with outstanding transportation opportunities. You will typically transport residents to medical appointments, dialysis, therapy, ambulatory surgery, outpatient services, and much more. You can even partner with Activity Departments to accommodate group outings for special events.


Dialysis Center:

There is probably no other source of revenue that is more predictable and stable than dialysis clients – bottom line. Because the vast majority of dialysis patients require treatment three days per week, regardless of holidays or weather, I refer to them as the “Golden Grail” because they are consistent money-makers!

For dialysis patients, treatments are mandatory – a medical necessity. Hence, underscoring why your medical transportation service is both weather and recession-proof. It doesn’t matter if it’s down pouring or if the economy is down, dialysis patients still have to get their appointments.

Many dialysis patients will be private pay while others are on Medicaid. Although the rates of reimbursement for private pay patients will typically be higher as compared to those on Medicaid, dialysis patients on Medicaid typically remain profitable. They offer volume and regularity with appointments being scheduled every other day in intervals of 3-4 hours. Such transports allow you to better plan and prepare your logistics and reimbursements.


Students:

Rest assured, non-emergency medical transportation is not just for the elderly. In fact, there are students of all ages needing your assistance to get to and from school, college, and university.
